This portable phonograph is located in the Office of Creative Incubation (fast travel point is the OCI Lower Exterior). You can reach it by heading into the maintenance room with a workbench on the second floor then climbing up the ladder inside the room. At the top of the ladder, head down the short hallway then take a right through the doorway. Facing the 3 terminals, take a right to find the shelf containing the portable phonograph on the right just before a giant hole in the wall.

This portable phonograph is obtained as part of the story. You will receive it in a parcel at the beginning of The Ones Who Walk Away from Gorgon main quest on the Unreliable. After interacting with the parcel, a cutscene will be triggered with the crew. When it is done, you can collect the phonograph from the table.
